  • I do not accept insurance at this time but will provide you with the necessary

       documentation for you to submit a claim for reimbursement. I encourage you to call
       your insurance company first to find out what your coverage is for an out-of-network
       provider.

  • There is a limited number of sliding scale spots dependent on need, please inquire for

        more information.

  • ï‚·Please cancel/reschedule appointments at least 24 hours in advance to avoid fees.

  • Client will be responsible for the entire amount of the session if cancelation or

        reschedule occurs after 24 hour notice.

  • ï‚·All clients must have a credit, debit or HAS card on file.

  • ï‚·Payment is expected at the time of treatment.
