I do not accept insurance at this time but will provide you with the necessary
documentation for you to submit a claim for reimbursement. I encourage you to call
your insurance company first to find out what your coverage is for an out-of-network
provider.

There is a limited number of sliding scale spots dependent on need, please inquire for
more information.

ï‚·Please cancel/reschedule appointments at least 24 hours in advance to avoid fees.

Client will be responsible for the entire amount of the session if cancelation or
reschedule occurs after 24 hour notice.

ï‚·All clients must have a credit, debit or HAS card on file.

ï‚·Payment is expected at the time of treatment.
